An exciting opportunity has arisen within the service for 3 newly created roles as Dementia Support Workers to work within our newly formed, Hospital Liaison & Community Intensive Support Team. Successful applicants will work alongside dedicated Senior Dementia Practitioners to follow a stepped care model approach as below:
1. Pre-admission escalation of service to provide intensive support with the aim of avoiding a hospital admission. This may include Dementia Care Mapping, carer education and support, use of flexi beds, etc.,
2. Inpatient admission with active liaison from the service ensuring all relevant parties have the required information held by the service
3. Facilitated discharge by the service ensuring that our team has made the necessary assessments for safe discharge and that the carer is both mentally equipped and physically able with the required skills to support the service user once discharged
4. Post discharge active involvement by the service for 2 to 3 weeks checking that the package of care is in and working, checking the carer is coping, checking medication compliance, etc.,
The role is Monday to Friday 10.00-18.00 and expected to work citywide based from Brookland Hall.
There will be opportunities for training and development as identified through supervision and appraisal.

Following an exciting service development an opportunityhas arisen within the service for3 x Band 6 Senior Dementia Practitioners to work into one of our 3 team hubs.
The role is focussed on supporting people with Dementia and their carers, families and staff working alongside the Dementia Navigator. Practitioners support GPs and/or Care Homes using a liaison model of delivery.
This is an opportunity for someone with a professional registration who will have transferable skills from previous experience working in Mental Health, Dementia or Physical Health and from a perspective which may be General Nursing, Mental Health Nursing, Physiotherapy, Occupational Therapy, Social Work or a variety of other professional backgrounds.
The role will necessitate working some occasional extended hours on our Access Point (Monday to Friday from 08.00-20.00) and will be flexible to work across the city of Bristol as required.
This service provides all staff with opportunities for excellent training and development, supervision and appraisal and not least to make a significant positive difference to the lives of people living with dementia.

An exciting opportunity has arisen at Symes, our South hub for a Band 7 Team Manager working full time. Following an exciting service development we have an opportunity for a Community Team Manager who will be accountable for the day to day operational delivery of the service from our South Bristol hub. The role will support and ensure the team engage proactively and positively with the local community both raising awareness of dementia and in providing excellent post diagnostic support.
